WAIVER AND RELEASE OF LIABILITY

I acknowledge that the attendance at, or participation in, any Nation, Federation or Tribal program or activity, and use of any equipment, facilities and transportation provided by any Nation, Federation or Tribal group, camp or facility could cause injury to me and/or my child(ren). In consideration of our registration and participation in a YMCA parent/child program, I/we hereby assume all risks of injury which may result from or arise out of my/our attendance at or use of equipment, activities, facilities or transportation for any program or activity; and I/we agree, on behalf of myself and my chiId(ren) and my/our heirs, executors, administrators and assigns to fully and forever release and discharge the Tri-Town YMCA, Two Rivers Federation and their respective officers, directors, employees, agents, successors and assigns and each of them (collectively, the "Releasees") from any and all claims, damages, demands, rights of actions or causes of action, present or future, known or unknown, anticipated or unanticipated, resulting from or arising out of my/our attendance at or use of equipment, activities, facilities or transportation for any program or activity, including without limitation any claims, damages, demands, rights of action or causes of action resulting from or arising out of negligence of the Releasees. Further, I and my child(ren) hereby agree to waive all of such claims, damages, demands, rights of action or causes of action. Further, I/we hereby agree to release and discharge the Releasees from any and all liability for any loss or theft of, or damage to, personal property. I acknowledge that I have carefully read this waiver and release and fully understand that it is a waiver and release of liability applicable to myself and my child(ren).

DRUG AND ALCOHOL POLICY

I understand that illegal drugs are not permitted and shall not be used at any campout or program event. If a Federation, Nation or Tribal event is being held in a place that does not allow alcoholic beverages, I agree to abide with that policy and not drink or provide alcoholic beverages. In the case of a campout, I understand that the Federation officers, Nation officers, Tribal officers or the camp staff may inspect the cabin and coolers for alcoholic beverages and illegal drugs. In the case of an off site event. (e.g. bowling outing, Christmas party) where alcoholic beverages may be served, I agree to put the interests of my child(ren) first, use moderation, and make sure my actions portray the YMCA programs in a positive light. If illegal drugs are found at any event, or if alcohol is found at any event that does not allow alcoholic beverages, a one year suspension from the program will be strictly enforced against the parent(s) and child(ren).

USE OF PHOTOGRAPHS

I acknowledge that photographs maybe taken of my child at Federation, Nation and Tribal events for promotional and informational purposes. I authorize the use of photographs taken of my child(ren) for such purposes, including their use on the web pages of the Tri-Town YMCA and/or the Two Rivers Federation.
